Through symptom management, cognitive care, emotional support, and fulfilling psychosocial needs, we aim to help our patients live out their final days in peace and dignity.

In addition to the hands-on services we provide for patients, we also assist their families. We can help provide access to the hospice benefit to ease the stress of the family during this difficult time. Our team also offers grief and bereavement services to provide the patient and their family with tools to navigate the end-of-life transition. These services do not cease when the patient passes, but rather continue for up to thirteen months following the passing of the patient.

An Interdisciplinary Team of Hospice Specialists

At Devotion Health Care Inc, we ensure that none of our patients are alone as they take life’s final journey. In addition to the support of family and friends, we provide an interdisciplinary team dedicated to ensuring each client’s needs are fully met.

Each patient’s team consists of the following individuals:

  • Medical Director: The medical director certifies and oversees the patient’s entire case and plan of care.
  • RN Case Manager: This is the patient’s primary point person. They oversee symptom management, medication management, level of care changes and navigation, and durable medical equipment (DME) support. They also provide physical and emotional support for the patient and their entire family.
  • CNA: A CNA will provide support for many routine activities of daily living and hygiene, such as bathing, changing, feeding, and more.
  • Medical Social Worker: The medical social worker is there to assist with the psychosocial needs of the patient and their family. They can help the patient and family sort out paperwork such as the advanced directive, POLST form, and living will, and they can assist the patient and family in putting their financial affairs in order. The medical social worker can also help arrange for respite care, placement in hospice general inpatient care, and DME support. However, their most important role is to provide emotional, cognitive, and spiritual support to the patient and their family.
  • Chaplain: A chaplain provides cognitive and spiritual support in line with the beliefs of the patient and their family. They are also available to provide grief and bereavement services and support after the patient has passed.
